Device Removal

1. How do I get my Ignition Interlock Device removed?

Individuals (not Service Centers) who need to have their Ignition Interlock Device removed should contact the Driver Improvement Bureau at driver.improvement@dps.texas.gov.

Registration

1. How do I apply for an IID certificate/license and what forms do I need to submit?

All applications should be mailed in with the required fees and the following forms:

2. What are the different types of IID Applications?

  • Original/New Application
  • Annual Inspection
    • Submitted yearly with fees and forms.
  • Renewal
    • Submitted every 2 years.
    • Expires on the second anniversary of the date of original issuance.

Moving or Relocating

1. What do I need to submit when relocating/moving my IID Service Center?

When moving or relocating, you will need to submit an IID Vendor's Information Change form (IID-02) and a new application (IID-01), including fees. (See TAC Rule 10.2.)

Application Status

1. How long does it take to complete the application process and issue a license?

Once the application and payment are submitted, it could take up to two weeks for processing. After the application and payment have been processed, a DPS representative will contact the owner to schedule an appointment to visit the location. During the visit they will conduct an inspection of the location, facility, and required equipment. If the required items are found to be compliant, the DPS IID team will issue a license number and email a certificate to the applicant.

2. How long does it take a DPS representative to visit the location and conduct the inspection?

Once the case is assigned to a DPS representative, it could take up to 60 days to contact the owner, schedule an appointment to visit the location, and conduct the inspection. 

Insurance Requirements

1. How do I submit insurance requirements?

For insurance requirements, submit a current Certificate of Insurance with the Service Center name and address and include the following limits: See TAC Rule 10.11.

Change of Ownership/Service Center Closure

1. What do I need to submit if the Service Center is changing ownership (i.e. Selling or closing the Service Center)?

  • A closure letter including:
    • Service Center Name
    • Service Center Number
    • Address
    • Signature

Adding, Removing, or Updating Owner(s)

1. What do I need if I am adding, removing, or updating ownership information to the existing Service Center?

  • Submit an Information Change Form (IID-02). This form can be used to remove an owner and/or add a new owner. If adding a new owner, the following forms must also be submitted:
    • Application for Authorization of Ignition Interlock Vendor (IID-01). This will be treated as a new application and will be issued a new Service Center Number.
    • IID Service Center Owner Information Form (IID-05)
    • Current Certificate of Insurance (See TAC 10.11)
