Licensing & Registration

The Department's Compassionate Use Program is no longer accepting applications for dispensing organization licenses.  The department has conditionally approved for licensure the top three scoring applicants. These applicants are now subject to onsite inspection requirements and shall submit payment in full of the license fee upon final departmental approval.

Should a conditionally approved applicant fail to complete the requirements for licensure, or decline to pursue the license, the next highest scoring qualified applicant will be conditionally approved.

Notice relating to Employee Registration Applications and Fees:
The department will now process employee registration applications from the conditionally approved applicants. Any employee registration application fees submitted by an applicant that is not conditionally approved for licensure will receive a refund of those fees.

Applications for director, manager, or employee registrations may be downloaded from the link below.
